Details of experience:
On 11/25/2025, Roy was admitted to our community, Water’s Edge, from St. Charles Hospital with an admitting diagnosis of Iron Deficiency Anemia, with chief complaints of right knee pain, loss of consciousness, fatigue, and repeated falls due to recent right knee destructive arthritis. Roy came to our community for comprehensive Rehabilitation and Skilled Nursing Care to recover and strengthen to return home.

Upon arrival, Roy was met by Water’s Edge’s interdisciplinary team, including Nursing, Concierge, Social Work, Recreation, and Therapy. Roy was evaluated by John Paul, Physical Therapist, and Erin, Occupational Therapist, who developed an individualized care plan to achieve his functional goals.

Roy required rehabilitation services to increase independence with his Activities of Daily Living (ADLs), improve dynamic balance, ambulatory ability, and confidence navigating steps. When Roy was admitted to Water’s Edge, he required moderate assistance for essential daily tasks, including dressing, bathing, and putting on footwear. Additionally, Roy was unable to ambulate 10 feet or navigate 1 step.

While working incredibly hard with our rehabilitation team to recover and regain his strength, Roy also immersed himself in the social and recreational activities offered by Water’s Edge. Roy was a frequent attendee of our live entertainment offerings, afternoon coffee socials, and card game tournaments, often being the first resident to arrive and the last one to leave.

After two months working with the incredible Rehabilitation team at Water’s Edge, Roy successfully met his Physical and Occupational goals, allowing him to safely discharge home. Upon discharge, Roy was able to ambulate 150ft independently with a standard walker, navigate 8 steps with a handrail, and achieve full independence in completing his Activities of Daily Living (ADLs).

Roy was deeply grateful for the care and support provided by the entire team at Water’s Edge and formed lasting friendships with fellow residents that still endure now. The entire team at Water’s Edge wishes Roy continued health, independence, and success in the next chapter of life.
